CERTIFICATION PROCESS

Quotation Request Form

On receipt of a query from the prospective client for fee proposal, QRF is sent to the client to get information related to no. of employees, the scope of certification, no. of sites with address, no. of shifts, etc.

Quotation

On receipt of filled in QRF, detailed Quotation (Service Specification & Agreement – SSA) is prepared and sent to the client for perusal. The agreement describes various services that Quality Mark agrees to provide to its Clients (including audit processes & certification processes for granting, maintaining, and renewing certification), the Client’s responsibilities, Fees, Terms, and Conditions of the Agreement. Additional information describing processes of expanding, reducing, suspending, or withdrawing certification shall be provided upon request.

Acceptance of Quotation

Clients are advised to review the agreement thoroughly and on acceptance of it are requested to send signed SSA, Application Fee, and Uncontrolled copy of system documentation to for commencing formal planning and scheduling date for Stage 1 Audit.

Stage 1 Audit

Documents are examined to establish general adequacy for compliance to assessment criteria (Standards). A program identifying the logistics of the assessment including sites and areas to be visited and approximate timing is prepared and issued to the clients.

Stage 2 Audit

A Comprehensive Assessment of the Client’s management System is undertaken and detailed non-compliance notes are raised where appropriate. These notes are called CARs within the Scheme. A summary report is also issued which includes a recommendation regarding the approval status at the time.

Certification Decision (Granting or Refusing Certification)

Based on audit conclusions, the Certification supervisor decides to grant certification if there is sufficient evidence of conformity or not to grant certification if there is not a sufficient evidence

Surveillance Activities (Maintaining Certification)

The Client’s Management System is audited at least once a year to ensure that the approval remains valid and that any changes to the System are examined for compliance with the assessment criteria.

Suspending, Restoring or Withdrawing certification of Expanding or reducing the scope of certification

Expanding or reducing the scope

In response to an application for expanding the scope of a certification already granted, We will review the application and determines any audit activities necessary to decide whether or not the extension may be granted.

We may reduce the scope of certification to exclude the parts not meeting the requirements when the certified client persistently or seriously failed to meet the certification requirements for those parts of the scope of certification.

Suspension and Withdrawal

Under suspension, the client’s management system certification is temporarily invalid. Failure to resolve the issues that have resulted in the suspension in a time frame established by Quality Mark results in withdrawal or reduction of the scope of certification.

Restoring Suspended certification

Suspended certification will be restored if the issue that has resulted in the suspension has been resolved.
